Blood Bank to get lots of cois + Jera can result in tons of coins. Jera in the Chest or Dark Room starting rooms result in twice as many items. It does have uses if used correctly. Round 1: Rune ShardThe only ever priority I can give you is unlocking the D6 (kill Isaac with ???) and then start playing Isaac. Apart from that, I think you should unlock the runes as they make the game more spicy (Jera, Dagaz, Algiz probably priority).
